Low-certainty evidence indicates that screening for breast cancer with mammography results in a modest reduction in breast cancer mortality for women aged 40 to 74 years; the absolute benefit is lowest for women younger than 50 years. The balance of benefits and harms from screening is less favourable for women aged 40 to 49 years than for older women. In light of low-certainty evidence for benefits and harms from breast cancer screening, as well as variability in patient preferences, women should be supported to make an informed choice on screening that is congruent with their own values and preferences; women aged 50 to 74 years should be engaged in shared decision-making with their health care providers.
